Fixed `sp', `br', `Pp', etc. so as not to have an empty div (-Thtml, -man, -mdoc).
Made html_idcat be completely correct (messy standard) (-Thtml).
Fixed warnings about -ohang and -item lists (-Thtml, -mdoc).
Fixed typo in index.sgml.
