Subject: Improve documentation of the header/footer macros .Dt, .Os, .TH: *
 State the defaults for .Os and the fourth .TH argument. * Sync the section
 titles, and stop advertising obscure sections that aren't actually fully
 supported and certainly not recommended for use.
